The interRAI Acute Care-Comprehensive Geriatric Assessment (interRAI AC-CGA) system for Community Hospitals, Territorial Hospital Services, and Geriatric Hospital Wards plays a crucial role within the broader interRAI Assessment Systems. It is specifically designed to meet the unique needs of elderly patients, as well as adult individuals, who face a range of complex functional and psychosocial challenges. Acting as a holistic and standardized tool, the interRAI AC-CGA Assessment and Care Planning System meticulously evaluates the diverse needs, strengths, and personal preferences of patients, particularly those facing significant functional and psychosocial issues. These problems greatly impact patient management in the mentioned hospital settings and the process of protected discharge from the hospital. Duration
